Just to wrap this up, I received a refund from ANA/Priceline about a month later. I was initially worried as the flight showed cancelled immediately after my phone call, but I neglected to take a screenshot showing such, and the second day afterwards the online reservation no longer mentioned cancelled anywhere. But I was pretty busy and waited awhile, and sure enough it worked out with the refund. As mentioned above, I got about 30% of my ticket cost back.
For the visa issue, still don't know why I was denied. The lawyer I used before didn't want to help. I am just waiting for now, even though I am paying rent overseas still. Will probably apply via the consulate at some point.
