Are there scenarios (besides loading issues, and deadheading crews) where there are seats available for purchase in First, and an MVP 100K is #1 on the upgrade list but yet not upgraded?

I cancelled a few of those but to be a bit more accurate, I'm flying transcon basically every other day for half
