Just looked through avherald.com and didn’t see any reported incidents.

Guess it’s good that these incidents are captured and reviewed internally, on the other hand, interesting that none of the events seem to be reported.

These sound like they came from pilot irregularity and/or ASAP reports, which are essentially non-public. Would be interesting to see the full memo, since some of the wording sounds like it was sent to QX management, not to the pilots. Nothing over on airline pilot central either.

I think this is another self induced crisis at QX. They let themselves get way too short on pilots and had to cancel a bunch of flights. So they react by hiring a ton of pilots, some of which were less qualified than what they would normally hire. Now they have a glut of pilots and some of them are not even getting enough flying time to maintain proficiency. That kind of my overall summary from what I read over on the APF.

This was exactly my thought too!

Curious what the pilots on our forum think of this.
