Irony on the matter is that on Monday I flew back to US w/o covid-19 test requirements and on Thursday I tested positive on covid-19.

There's a new version of the CDC attestation form that's apparently required for all non-citizens (of the USA) and nonresidents (???) flying to the USA. You can print it from the CDC website (google).
