Incident: Air Canada E190 near Toronto on Aug 2nd 2011, pressurization problem

An Air Canada Embraer ERJ-190, registration C-FHKS performing flight AC-177 from Toronto,ON to Edmonton,AB (Canada) with 98 people on board, was climbing through FL270 out of Toronto when the crew declared PAN PAN and initiated a rapid descent due to pressurization problems. The aircraft returned to Toronto, burned off fuel and landed safely on Toronto's runway 23 about 2 hours later.

The Canadian TSB reported the aircraft had been dispatched under minimum equipment list requirements with the #1 bleed air system inoperative. At FL270 the crew received a #2 bleed system fault message, declared PAN PAN and returned to Toronto. Maintenance replaced the #1 Torque Motor Controller, #1 Bleed Pressure Sensor, the #1 high stage bleed valve and the Secondary Power Distribution Assembly 2 Air Management System Controller Input/Output Module. Checks of filters and leak checks of lines were performed with no fault found. A subsequent operational and leak check also found no faults permitting the aircraft to return to service.

Incident: Air Canada A333 at Toronto on Aug 4th 2011, taxiway excursion

An Air Canada Airbus A330-300, registration C-GHLM performing flight AC-876 from Toronto,ON (Canada) to Frankfurt/Main (Germany) with 265 passengers, was cleared for takeoff from runway 06R at 22:35L (02:35Z Aug 5th) while taxiing along taxiway D when the left main gear left paved surface disabling the aircraft on the taxiway still penetrating the protected runway 06L area.

The airline confirmed the aircraft accidentally taxied into the grass. The passengers have been taken to hotels and are going to be rebooked onto other flights on Friday.

NAV Canada reported the aircraft taxied off taxiway D into the grass short of runway 06R with the left gear on the grass east of the taxiway.

The crew had been unsure of how to proceed to taxi along taxiway D asking whether they should turn left or right or continue straight, with the tower replying they should continue straight ahead, a left turn would take them on taxiway D6.

Incident: Air Canada B763 near Bristol on Jul 19th 2011, oven choked over meals

An Air Canada Boeing 767-300, registration C-GHLA performing flight AC-889 from London Heathrow,EN (UK) to Ottawa,ON (Canada) with 208 people on board, was climbing out of London Heathrow near Bristol,EN (UK) when an aft galley oven began to emit smoke. Cabin crew switched the oven off and the smoke dissipated. The flight was continued to destination, the oven remained off for the remainder of the flight. On approach to Ottawa the crew reported a medical emergency on board and continued for a safe landing on Ottawa's runway 25.

The Canadian TSB reported the source of the smoke were burnt meal casseroles.

Incident: Air Canada A320 at Vancouver on Aug 5th 2011, flaps locked

An Air Canada Airbus A320-200, registration C-FFWM performing flight AC-109 from Toronto,ON to Vancouver,BC (Canada) with 150 people on board, was on final approach to Vancouver's runway 08L when the crew went around reporting a flaps problem. After working the relevant checklists the crew declared emergency and landed safely on runway 26R 12 minutes after declaring emergency.

The Canadian TSB reported that the crew received a "F/CTL FLAPS LOCK" ECAM message. Maintenance found the flap interconnecting strut out of rig.

Accident: Air Canada B763 near Vancouver on Aug 8th 2011, fumes in cabin sicken cabin crew

An Air Canada Boeing 767-300, registration C-FCAB performing flight AC-63 from Vancouver,BC (Canada) to Seoul (South Korea) with 221 people on board, was climbing through FL200 when cabin crew reported fumes from the air conditioning system. The captain turned one of the re-circulation fans off, consulted with maintenance, and while already enroute at FL300 decided to return to Vancouver when cabin crew began reporting dizziness, sickness, pale complexion and hypoxia. Due to the pending overweight landing and the possibility of overheating brakes the flight crew requested emergency services on stand by for the arrival. The aircraft landed safely on Vancouver's runway 26L about 2 hours after the onset of trouble and about 75 minutes after the decision to return.

The Canadian TSB reported the affected cabin crew members were sent to the hospital for medical attention. Maintenance cleaned filters and sensors of the air conditioning system and galley and returned the aircraft to service.

Incident: Air Canada E190 near Montreal on Aug 15th 2011, smelly coffee

An Air Canada Embraer ERJ-190, registration C-FHNX performing flight AC-682 from Toronto,ON to Deer Lake,NL (Canada) with 82 people on board, was enroute at FL390 about 100nm westsouthwest of Montreal,QC when the crew declared emergency reporting an electrical fire on board and decided to divert to Montreal. Cabin crew had noticed smell of smoke and subsequently identified the source to be near a number of coffee makers and turned them off, the circuit breaker was pulled as well. The aircraft landed safely on Montreal's runway 06L about 18 minutes later and stopped on the runway.

Following examinations the aircraft taxied to the apron, after a runway inspection the runway was re-opened about 15 minutes after landing.

The Canadian TSB reported a coffee maker in the forward galley was found defective with two wires to the tank being burned. The coffee maker was replaced.

Passengers also reported that Air Canada’s flight 942 from Toronto had taken a hit from lightning as it approached the Island.

Incident: Air Canada B763 at London on Aug 12th 2011, rejected takeoff

An Air Canada Boeing 767-300, registration C-FOCA performing flight AC-859 from London Heathrow,EN (UK) to Toronto,ON (Canada) with 218 people on board, rejected takeoff from Heathrow at approximately 90 knots when the first officer's air speed indication was erroneous. Emergency services checked the brakes before the aircraft taxied to the apron.

The Canadian TSB reported the analog air speed indicator was replaced, the rest of the system was verified and operated normally.

Accident: Air Canada E190 enroute on Aug 22nd 2011, too hot a coffee

An Air Canada Embraer ERJ-190, registration C-FHJU performing flight AC-665 from Halifax,NS to Montreal,QC (Canada) with 93 passengers and 5 crew, was enroute at FL380 when a flight attendant received injuries due to hot coffee and was unable to continue duties. The aircraft continued to Montreal for a safe landing about 40 minutes later.

The Canadian TSB reported the flight attendant received second degree burns from the coffee.

Incident: Jazz DH8A near Vancouver on Sep 9th 2011, hydraulic failure

An Air Canada Jazz de Havilland Dash 8-100, registration C-GKON performing flight QK-8064 from Victoria,BC to Vancouver,BC (Canada) with 31 people on board, was on approach to Vancouver when the crew reported hydraulic problems and entered a holding. After about 15 minutes the crew continued to Vancouver for a safe landing on runway 26L, vacated the runway and was towed to the apron.

The Canadian TSB reported the Dash 8 had lost the #1 hydraulic system causing the loss of flaps and brakes. The aircraft stopped after vacating the runway and was towed to the apron.

Seems that they had started their descent into YQR (dropped to FL220), and then, wham, they turn around, climb back to FL310, and land in YWG 35 minutes later.

Don't see anything on CADORS. After being at YWG for an hour, they took off and flew to Regina, so anyone have an idea what happened?

Accident: Air Canada B772 enroute on Sep 19th 2011, turbulence burns 6

An Air Canada Boeing 777-200, registration C-FNNH performing flight AC-33 (dep Sep 17th) from Vancouver,BC (Canada) to Sydney,NS (Australia) with 269 people on board, was enroute, cabin crew serving coffee and tea, when the aircraft encountered turbulence causing coffee and tea to leave the cups unexpectedly and spill over passengers and crew causing minor burns to 4 passengers and 2 crew. The aircraft continued to Sydney for a safe landing on runway 34L, where an Australian news helicopter filmed the arrival. Nobody needed to be taken to a hospital.

The airline confirmed 4 passengers and 2 crew received minor injuries from spilled beverages and were attended by a doctor on board.
