It sounds like user error to me

No, you've got it wrong, you are still supposed to consume, just not be successfully checked in. I've been sneaking past the gate keepers when they are distracted.

How are 'stops' handled? (not official connection/segment in the Altitude sense but technically a landing/takeoff)
ie same flight number?
Reading the rules it says any takeoff and landing qualifies as a badge, so shouldn't a stop count?
Is this situation covered specifically elsewhere in the rules? I didn't see anything.

One flight # one ticket counted once: one takeoff/landing only at the ticketed origin/destination
One flight # on separate tickets posted to Aeroplan and EYW as two separate flights, so both takeoffs and both landings.
One flight # in SPLIT cabins (Y from a-b, PY/J from b-c) posted as two separate flights, so both takeoffs and both landings.
