Initially yes, but after downloading my activity and checking against my excel tracker I have two trips with pending AQD from dec 5 and 10th which should put me over by $300.

Still not 100% 'working', though. Every AQD entry in my list is added as a separate flight with 0 AQM attached, so e.g. a P fare on AC has three entries in the list: base AQM, CoS AQM, and AQD.

Yup, and this was at the beginning of December. Each flight took 3 lines. It wasn't 3 per flight on the November screen shots.

That used to be the case but that changed (not sure when) - here is the language from the Altitude site "Early Recognition is offered for all tiers including Prestige 25K".
