Air Canada medical clearance for supposed mask non-compliance

Recently flew and husband had an interesting incident and not sure what to do.
He was helping our daughter by carrying all her and our 1 year old granddaughters luggage onto the plane. His mask slipped off his nose, was told and corrected it right away. He then held our grandaughter so our daughter could get seated and the one year old grabbed his mask again slipping it off his nose and again he corrected it right away. The stewardess then handed him a card that says he needs medical clearance and will be placed under a travel restriction. We are all very compliant and have no issues wearing a mask and he is a doctor so knows how to wear one. This felt extreme especially since teenagers were wearing there's below the nose, another guy was and another man kept taking his completely off and even went to the washroom without it on. The same stewardess just reminded him to put his mask on. We are so confused and not sure what our next step is. It says his travel is restricted until he fills our a medical clearance form. This is crazy! Anyone experience this and what did you do? Thank you.
