I've got this offer in my Le Club AccorHotels email. I have done the instruction below and received email to get a luggage tag

I installed the app too but have received nothing.
I have a few A380 sectors coming up in the next couple of weeks, I'll see if that spurs anything on.

I received this e-mail about 2 months ago. The whole process worked fine and I received the luggage tag a few weeks after.

I can't find where to order the tag. The instructions aren't very helpful, it says "Create your profile with your Le Club AccorHotels address." Does that mean the same e-mail address you use for LCAH?

Just got the tag. Well, two tags, actually - for some reason they sent me two, even if I only registered once. The address labels are slightly different, my family name was written on uppercase letters on the second one.
Looks like the interactivity is the QR code and the RDIF chip on the card. Scan the code / chip and it takes you to a page on moustag.com, where the finder can contact you.

Just back from a trip to Aus, 4x A380 sectors, tag was here when I got home. I had logged the trip in the app, whether that made a difference or not I don't know. Flight info on the app was pretty useful and it's free.
