As you may recall back in 2001,
http://www.dash.com closed it's doors. At that time they posted a message that any prior purchases made to the June 22, 2001 closing will be honored.
They instructed us to send in the prrof of purchases to a particular address in NY and mileage will be credited within 6 months.
I contacted M+ to see where my 2,000+ miles are. They told me to fax the information in and if all was legit, they would credit.
I received a return e-mail back:
