Yes. I've done it several times.
On the phone, the absolute minimum they need to issue an add'l card is a name and a birthdate (to ensure the person is >= 18).
If they ask for the SSN of the add'l cardmember, just say you don't know it. That's all I've needed to say in the past.
You (the main cardholder) will be responsible for all charges on the additional card, of course.
