I had the exact same problem with 500-mile e-upgrades recently; I had an upgrade confirmed on a flight that took a long operational delay so I was rebooked on something else (and confirmed in F)...and they basically drew upgrades twice. Called MPCS, told to wait 72 hours...the upgrades did not redeposit. So called back, they put in a manual request. One week later, the upgrades reappeared in my account.
