I got the same snail mail letter - looked official enough - so didn't raise any phishy issues with me.

However, the OP reminded me that I haven't actually signed up for the offer yet and the letter/form is at home, but I am currently sat at LAX waiting for NZ5!

If someone finds the URL can they PM me it so I don't miss out on double miles (mmmm).
