Except in London where, as I have just discovered, CX offers complimentary limo service on all published First & Business fares including Round The World Fares... (but not including award fares, except that paid business mileage-upgraded to first would qualify presumably).

From cxagents.com:

Code:
London Courtesy Limousine Service
	Last Update: 20Mar2006

1. The Service:
The limousine service is available for customers travelling on Cathay Pacific
Airways flights, departing and/or arriving at Heathrow Airport, within 40
road miles.

2. Eligibility:
The service is complimentary to customers holding the following tickets:

    * First Class Published & Round-the-world Fares.
    * Business Class Published & Round-the-world Fares.

3. Excess Mileage:
For clients who exceed the free mileage but wish to use the service by paying
the mileage excess, the rates are as follows:

    * GBP 2.20 per car/mile (for normal cars)
    * GBP 2.86 per car/mile (for people mover*)
    * "Burgundy Global Ltd", the limousine provider, will only accept
credit card as in-car payment, drivers do NOT accept cash or debit cards
e.g. Maestro/Switch.

4. How to Book the Limousine Service:

    * Via Email - [email protected]
    * Via Fax - 020 8741 5477 - To the Servicing Department
    * Via SSR Request - Enter SSR request giving full address and telephone
number for the client, and Corp. client if applicable

* People mover is like a space wagon and is ideal for 3 passengers (min) /
6 passengers (max) who wishes to travel together. Reservations should be made
at least 48 hours prior to departure.

*It is compulsory as legal requirement to all passengers traveling with
infants to bring their own car seats. "Burgundy Global Ltd", the limousine
provider, does not provide car seats.

Important Notice: There are surcharges of 100% on excess mileage on
25 December, 26 December and 01 January and surcharges of 50% on all other
public holidays.
