I’ve had this happen a couple of times in fairly recent memory (last three years)

FAI-ANC-PDX on AS
PDX-SLC-SAT on DL

There might be a few other examples that are slipping my mind. In general I would say it’s likely to happen from time to time to any frequent flier.
