Yes -- The SF-95 is for property damage caused by the government and is a one-size-fits-all form. ("SF stands for "Standard Form.") I've filled them out for damage caused by movers during military PCS moves. Years ago, I remember a conversation with an Air Force JAG who told me that about 20% of all claims are never filed because the filer is intimidated by the complexity of the form.

If the screener or the supervisor told you to fill out an SF-95 to make a complaint, I would add that bit of misleading information to the complaint.
