A second alternative is to book the cheapest possible flight you can find (does not have to realistic -- a short sub$100 flight) and you will be issues credits (residual FFC) that can be used by anyone
i know a lot has changed, but IIRC, residuals now go to an FFC, which is for original traveler only. Pre-pandemic, they would go to ETC (useable by anyone), and for a while, they allowed the residual FFCs for use by anyone, but I thought that was only when the change was made by a certain time (August, IIRC). The medical route is probably the best option if available. Or maybe UA would grant an exception on request, based on details of situation, to convert into an ETC.
