I honestly don't think there has been a way to request conversion of an offline FTV to an online evoucher ever since FTVs started. I am open to the possibility of being wrong on this, but I am pretty sure this is correct. I also had a look through this thread for end of November through December and the only post with a link to something is this one

If it was this one

If you have a offline FTV then you can still use it, but you need to call to make the new booking.
