Here is the answer as of Jan 2020:

a) No need to send original US passport for Re-Issue of OCI card
- Need "Affidavit in Lieu of Original" form notarized
- Copies of 1st page and Endorsement page(s) (even if blank) need to be sent

b) Unfortunately, original OCI booklet (officially called OCI Card) DOES need to be sent
(which means you cannot travel while "Re-Issue" is taking place)
