I called in and they told me just to send in a picture of my passport and no mention of a form.
Have a feeling it probably won’t work out for me because when I created my account initially I got an error with the passport field, so tried again without the last number of my passport number which worked. So made 10 bookings with that IB number and afterwards I realized I got a confirmation email of another account being created which I assumed is the one with my full passport number.
So I explained the situation in my email to them with my passport photo, so hopefully they’ll let it slide.
