Do you work in media (or another hot button industry) by any chance? If so, listing a side job on the application (or even self employed) might have helped.
For my mom's recent visa application, I made a second flight/hotel reservation 8 months out for good measure. This costs nothing, and pretty much ensured she would get at least one year. She got ten years.
