The flight can leave from T4 a few days, then TBIT, then T4, then a few days from TBIT... I would estimate about an additional 15 minutes if the flight leaves from TBIT.
T4 versus T5: More than once I have made the error of boarding the shuttle bus to the gate / terminal shown on a BP created several hours prior... only to find AA has done a late gate change and I have bussed to the wrong place. Mistake adds another 10+ minutes in the underground Austin Powers stuck-cart walkway.
