While what you describe could have happened, it's just seems like an unlikely coincidence that the other tag would be to BCN when going to MAD.
A passenger's tag can only have the wrong final destination if the bag were being checked on an itinerary with separate tickets. The agent has to manually type the other ticket's flight number and destination city. It's impossible for a tag to have the wrong final destination for itineraries one one ticket without any connections exceeding six hours.
