Most ticket counter positions share bag tag printers in pairs. When both agents are printing tags at the same time, one might grab the other agent's tag by accident and tag the bag without checking the name and destination on the tag. Or, the agent can pull up the wrong PNR and print a tag in the wrong PNR and apply it to the bag.
It's surprising when a passenger arrives at the final destination baggage service office without a bag and they present a baggage claim check with someone else's name and itinerary info but the passenger never noticed the discrepancy. I saw that happen one time. The good thing about tag swaps is it's usually easy to reunite the bags with their owners.
