I'm pretty sure it was Transavia, at least the AMS-LGW-IAH codeshare was in 1999.

I don't remember Martinair DC-10s operating for CU just the 3 AOM ones including the one bunted at Guatemala City. But since they never bothered to paint any if them in CU livery, it's hard to tell.
