You could try loading an amount equal to BB's minimum ($20) to see if it goes through. If it does you'll know the problem was limit related. If you've always loaded BB with exactly $500 every time, then it's pointless to try the test.
I'm not sure if the monthly cap is a rolling limit but I'm sure someone here does.
