I have a bit of information to add to the information on international aircraft: The 757-200ER (with BusinessElite) also seems to be used on the JFK-GEO route, admittedly a somewhat obscure one.
I also have a question related to this: Do you know what the J seats are like on that aircraft? Are they more like domestic BE seats than international? (I realize they are not flat-beds or lie-flats) Do you get a duvet and larger pillow?
Medallions are eligible for complimentary upgrades on the JFK-GEO route, which makes me pessimistic about the quality of the BE cabin!
The 752ER has cradle BE seats with footrests, although they are different than the ones on the 763ER. The website says there are pillows and duvets:
