EDIT: ULTIMATE REWARDS, not Membership Rewards
Not directly but recently (10 weeks ago) opened a bank account with Chase (checking) when I moved to Indianapolis. Anyway, they said when your first payroll check goes in, we will send you a 125$ sign on bonus. Lo and behold on Monday a tax document from Chase comes through asking me to submit with my Tax Return. So I know not exactly what you are asking but Chase are on the ball with matters of this nature IMO.
