I have US- and Canadian-issued CCs. I'm trying to buy a ticket ex-KHI, but the purchase fails. EK call centre says the fare is "sales city restricted" and can only be purchased in Pakistan - which the website translates as can only be purchased using a Pakistani-issued CC. Of course, I could also have someone pay locally in Pakistan (but then can not use the 10% coupon for local purchases, and the fare is marked up an add'l 7%).
