BA charges 105K miles, not 75K miles, for a roundtrip ticket in First between Hawaii and the mainland.
And AFAIK, when you redeem AAdvantage miles for flights on AA metal, there are no fuel surcharges, even when you fly on routes where AA has incorporated a fuel surcharge on its cash fares.
