I'm guessing they want a copy or scan of the card, not the piece of plastic. So, do a Google Image search for "Aadvantage platinum card" (substitute appropriate level) and save the first hit. With the graphics or photo editing program of your choice, replace the number and name with your own, and update the expiration date to 02/11. Save, print and photocopy.
It may be dodgy, but if you really have the status and the number is really yours, you're not cheating anyone.
