Both my daughter and I have experienced the occasional dairy vegetarian meal when a non-dairy vegetarian meal was selected.
Unfortunately, the problem usually resides with the local catering vendor. In each of those instances, the FA has put together a special plate of items from unused trays.
Now and then I have had some superb meals in BE on TATL flights but I had one recently that consisted primarily of something green and looked like an ice cream scoop molded it.
Overall, I think DL really needs to review their offerings in this area.
