Ok good and bad news. The form seems to work great with my Android phone... not so good on my Blackberry. Looks like you are using something different so I can't guarantee it will work.
With the BlackBerry--would there be a difference between whether the browser is configured in "page view" or "select view" as well as some of the other options--like XML/WML support?
