BingBongBoy and Stez: the online application asks for:
Annual Personal Income * Your income before tax, plus any income from investments. Please enter in numbers only, e.g. £20,000 should be entered as 20000.

Annual Other Household Income Your partner/spouse's income before tax. Do not include your Annual Personal Income.

So, it's your gross income (before tax) plus investments. I think it should include allowances and the like. Given that others' perks such as company cars and private health insurance are PAYE-taxable, they can be considered income. I'm not sure whether the notional value of ID90s should be included.
