They didn't charge for it and if you read some threads over at the UA forum some people reporting getting charged 20$.
Also I did not need to send a letter, I just called them and they relayed the message. Came in about 5 days.
But I am GS with UA so this might have to do with getting no charge and also not having to send a letter.
