Doesn't happen often. Last time it did for me was a Caribbean flight out of CLT midweek on business during the height of hurricane season - definitely not high season to the destination (I think I was heading to STT or BGI).
I've had two but they where a long time ago. DEN-CLT and MIA -PIT. The MIA flight was a 737-200 and I had two omelets as it was a morning flight. The DEN-CLT was a 737-300 and I had a lovely experience with a full bottle ( or more) of a Pinot noir from Oregon.

There was no F but I flew a US Airways Express Dash 8 awhile back around the 4th of July from ABE-DCA and I was the only passenger on board (other than the flight crew of course)...it felt like having my own private plane
On another weird note...I flew a US Airways Express flight out of Reading, PA when all the passengers were male...weird....remember when US flew from Reading? Ah, the glory days The flight to PHL had to be one of the shortest in the system (unless you count the Lancaster-Reading segment that was added to some of the flights).

Many times, actually. Though I think it has been a couple years since it happened on US -- LAS-DEN, I believe.
LAX-NRT in ANA F all alone about two years ago was probably the highlight -- though pretty surreal having the whole cabin, and staff, to myself. I wasn't sure which meal to choose, so they suggested they bring me a sampling of several and I could choose. Or have them all.

But as my posted F trip reports prove, you can just about always have all the meals when seated in international F!!!

In 2008, when US was flying the A330-300s (back then still configured with row 1 as the former international F cabin) between PHL and SJU, I did a mileage run a weekend and I ended up being the only pax in both directions in the row 1 mini-cabin, which back then US only allowed CPs to reserve in advance.

Never happened to me... The closest experience I had was on BA in F LHR-ORD - 2 FF redemption passengers plus I. The F purser was the best I have ever seen on BA. Wonderful experience.

My and my partner had the F cabin to ourselves on an LH A340-600 MUC-ORD. I remember the purser introduced herself to us after we boarded and told us we were the only passengers in F on this flight. She went on to say "consider this cabin your own personal living room over the Atlantic"
We are both vegetarian and LH boarded the wrong special meals. They still setup the caviar cart and brought it out, even though we don't eat it. We ended up with pasta from C class as our entree. I remember some of the other F courses having vegetarian options. The FA's setup a wine tasting for us, which was fun. I think we sampled everything they had on board.
