Wheelchair assist LHR T3 to T4?

My sister-in-law is flying from the US to Malta via LHR next month. She has requested wheelchair assistance at the airport but because of her AA to KM transfer she needs to claim bags and recheck in a different terminal. Any experience in how this might work for her? I assume that the inbound assistance will get her through customs and perhaps into a taxi or the train, but that she will be on her own to get from there to check-in at T4. Any info/advice would be much appreciated.
