Interesting thread. I only have a few to list.
YCD Nanaimo, Canada - flew in only. I left in a car with friends to the mainland and flew home from YVR.
DTW Detroit - flew out only since I was visiting my parents, but arrived via a long road trip. Then went on a two month trip with multiple stops in SE Asia, came home to YEG by a different route.
SJZ Velas, Sao Jorge Island, Azores, Portugal - flew in only, left by ferry to Pico.
HOR Horta, Faial Island, Azores, Potugal - flew out only, arrived by ferry from Pico.

Not many compared to most here...
OAK -- back in the 1990s, flew in via OAK, was late to my return flight, next flight for standby was out of SFO. Funny part is I'm local now, but with hardly any AA service out of OAK I've still never departed from it.
MHT -- had a round trip, flight got cancelled; AA (back when service was still good!) paid for a cab to BOS
FCO/ZRH - in one, out the other.
REP -- flew in from KUL when my wife and I visited Angkor Watt, ground transit out because of much higher cost going straight to BKK.
HND -- Had a round trip booked, and changed my ticket such that I was departing Narita instead.
edit: missed one:
MFM -- flew in once, took ferry to onward flight out of HKG
Tenuous, but if we don't count transits, I can add:
* KIX -- I've arrived for transit once as well, but had it as a destination. I have have flown out of Japan from KIX.
* DEN -- connected a couple of times on United, flew out once (car died on a cross-country trip) but never flown in.
Since someone else mentioned, transit only:
* DTW - flew through several times back when I flew NW, never left the airport
* PDX - connected on a mileage run (SFO-LAX the long way), never left airport
* ATL - connected once each way on DL years ago (only time I've ever flown DL!), never left airport
* PHX - connected several times on US and once on post-merger American, never left airport
* STL - connected a few times on AA, never left airport
* CLT - connected once on AA, never left airport (of US places on this list, the only one have never been to at all outside the airport)
Tarmac-only:
* BNE - flew a through itinerary HKG-BNE-CNS, didn't have an opportunity to deplane.
* MDT - diverted for a fuel stop during an in-air weather delay, didn't have an opportunity to deplane

I've actually been booked into GPT twice in the last couple of years but have yet to land there! First time I was diverted to BTR due to fog and bused to GPT. Second time my originating flight was delayed due to crew issues and I was going to miss my connection to GPT. They were hard-pressed to get me to GPT that day so I flew to MOB instead. These were open jaws, so I never planned to fly out of GPT.

I was a senior in HS and was visiting Cornell, as I had applied there. Was supposed to connect in PHL to ITH, but was sent to ELM instead due to weather. They bused us to ITH. After my visit, I flew out of ITH.

Visited my sister in Columbus, OH. Have flown into/out of CMH multiple times, but this time, ended up having to return home out of DAY for some reason I can't recall. Only time I have ever set foot in that airport.

My list is probably pretty short, and off the top of my head my list looks something like
- Arrived VIE, land tour, departed ZRH
- LIN and FCO via US-LHR // LHR-LIN // FCO-LHR // LHR-US
- I've flown into DCA but don't think I've ever originated there
- have connected through and arrived at JFK but never originated
- least certain of this one: pretty sure at some point visiting family we flew a one-way either into or out of KHH. It's also possible the other end of this flight was TSA and would also qualify.
and I have been both airside (connections) and landside (dropping people off) at LAX but never gone thru security nor exited there.
EDIT: have also been here a few times, but only by car and only for lunch

So the common pattern is that it is possible with countries/regions with well developed overland public rail/bus transport and where airports are fairly close to each other.
