New York Air -- you got a bag with a bagel in it as part of their "Flying Nosh."
And Piedmont, which always had friendly crew. My brother and I would fly BOS-DAY-FNT and back to visit our grandparents and try to collect a pack of playing cards per person on each flight.

I have fond memories of EA as a kid in the late 70s and early 80s, although it may have been the cockpit tours, playing cards, and free wings every time I joined my mother on a biz trip.
I also enjoyed NW. Although I only flew them 30 or 40 times, I can't recall a bad flight and they handled IRROPS very well.

TWA, first class on an L1011 when that was a somewhat glamorous experience.
Not a defunct airline, but a defunct service: AA's all-F Fokkers that flew out of Love Field to kill off startup competitors in Dallas. Not just F seats, but an enhanced F service, more like int'l J than domestic F. Nice meals, a unique winelist just for that service, etc.
