This is very easy to have happened. Eventually, I've been to most cities that I usually only connect through. Exceptions for me include Tamp, Minneapolis, and Salt Lake City. Iv'e been to about 100 aiports.
Outside the US, big omissions include Frankfurt and Taipei. I've also never been to or through Dubai, Rome, Moscow, Kuala Lumpur, or Mumbai.

I had an overnight in Detroit years ago when a Northwest Pilot's job action cancelled a flight. I remember seeing the Ford Museum in the dark as the cab drove by to my hotel where I slept for 5 or 6 hours before heading back to the airport.
I once had a medical emergency landing in Little Rock.
AKL, LAX (though that's about to change), PHL, NAN, BHX is my list, though frankly LAX is the only one I've been to a few times and never ventured out of the nearby hotels.
Before September, I would have said HEL, as I did the LHR-HEL-JFK connection a couple of times without leaving Vantaa; however, I finally got to visit Helsinki itself two months ago (and had a great time)

Only two: IAH and ATL.
Some cities I've connected through, I've visited only later on a road trip. Oddly, that was the case with LHR--I connected twice through LHR (two years in a row) on the way to MUC, and on the second trip, picked up a friend's car in MUC and drove back to London via Austria, Hungary, Slovakia, Poland, Lithuania, Latvia, Estonia, Finland, Sweden, Norway, Denmark, Germany, The Netherlands, Belgium, France, and Ireland.
