had my first double denial on trip insurance claim recently..
Was pickpocketed on a trip to Russia recently, ran up $300 in cell phone charges calling back to the US to cancel my cards, etc. They said they won't cover those phone charges. well I guess I can live with that.
also had a flight I'd purchased Kaliningrad - Minsk. The airline cancelled the flight and rescheduled me on a flight the previous day. Well my visa was only good from the original day so that didn't work. Had to purchase another flight for $200 more (still waiting on a refund from the original flight).
I guess the 2nd one is sticky as well; I wasn't delayed but the airline did cancel the flight (but ~8 days before the flight). What's the point in trip insurance then if they don't cover flight cancellations?
Sorry to hear your disappointment. The typical add-on "trip "insurance" stuff is usually so loaded with fine print and weasel-worded phrases I totally avoid even looking at it. I have purchased some of the many insurances sold by AMEX, have had a few claims and was satisfied with with the results ie positive to me. My advice is stick with the majors, avoid the regular add-on stuff and download and read the policy before purchase.
Were either of the things you submitted claims for listed as covered items? If they were not, then there was probably not much hope in getting coverage. Travel Insurance is pretty specific, it covers exactly what is listed and not usually anything more.
I think most confusion with travel insurance is people buy a policy without having any clue what is covered or what is not, if it's primary or if it's not or any of the restrictions on the policy.
The point of travel insurance is to cover the items listed in the policy. If it's not listed (and I don't think many, if any cover the airline changing their schedule) it's not going to be covered.

Yeah.. like I said the phone calls I can live with I suppose.. they will cover identity theft charges, but not the charges to ensure that there aren't any more identity theft charges (the pickpockets made $1k in purchases even though I called cards immediately, hence the $300). But my bank covers that anyway. Out $450 there (calls and $$ in wallet).
The flight cancellation is an odd one too though, if the flight was delayed, I'd be covered. but if they cancel the flight entirely, I'm not covered. The airline cancelled flights for several days on either side (and there was only one flight per day). I was flying back home out of Minsk so didn't have any other choice but to buy an extra ticket to get there. Out another $200 there.
