FWIW, I signed up for the $25 card. I called in to ask a few q's and the agent noticed I had the regular card, not the preferred $65 card. Agent warned me that this promo is ONLY good for the $65 card - the $25 card would yield no bonus. He pointed me to the tc's which he claimed are "specifically worded" not to include the $25 card. So, I dropped down to the free non-United card at his recommendation
. Now it remains to be seen if I'll even get the 10,000.

The only way I know of is to visit an area where Chase has branches. Happened to be in AZ last week and opened an account there. Made my first purchase and a direct deposit will be made this Friday. Then it is waiting game for the points to post.
If you donot have an opportumity to visit Yuma for a Chase branch, all you got to do is wait for a few weeks. I was told by my friends at WAMU that it will become Chase starting April 1. So Chase is coming to your neighborhood in Cali soon.
That is correct-however I would wait as the WaMu branch reps have no idea about Chase products such as this one.
There will be some pickup time on this.

Unlike Citi, I believe Chase will charge it as Cash Advance. Not 100% sure though. Never tried it with Chase personally. Only read it somewhere online.
Did anyone actually receive the full 10000 bonus miles for the direct deposit? I opened my account back in Febuary and funded the account with a dd in March. I received the 5000 bonus for the debit card almost immediately following my first purchase. But I only received 2500 miles for the actual checking account so far. Anyone else in the same situation?
I got my 5K for the debit card but nothing for the checking account even though I had a DD done on my paycheck. Maybe, the promotion is only for the $65 card and not for the $25 card (which I opted for), as someone has pointed out in a previous post. If that be the case, the days for keeping this account open are numbered. Anyone have luck with receiving the 10K recently?
Just noticed the following in their T&Cs:
"The first 10,000 bonus miles will be credited to your Mileage Plus Account 6-10 weeks after the initial direct deposit has posted to your new account."
Maybe, I need to allow more time for them to follow through on crediting my account for the 10K award.

I bank at a Calif. Wa Mu branch. Although all of their signs say Chase and they wear Chase shirts, they said they cannot open a Chase account until October 2009. California is the last state to be fully integrated with Chase. If you go to the Chase.com website and give a Calif. zip code, you will be redirected to a Wa Mu product.
Having said that...
Today I went into my local WaMu/Chase branch with a copy of the offer and coupon. They called a Chase branch in Arizona and opened my account there. They faxed my ID and the coupon to the Ariz branch and the Ariz branch is faxing them the signature cards for me to sign this afternoon. I already saw the Chase account when I logged into my Chase MP Visa account a few minutes ago.
Someone at the Arizona branch just called me to get my united MP #, so that would be a good thing to bring into the local calif WaMu/Chase branch with you along with the coupon fo them to fax.
In addition to the 15,000 United bonus miles, I am interested in this account as I can make tax payments on the Visa Debit card and get 1 United mile per $1 spent without the 2.49% fee charged by Official Payments for tax payments. OP is now only charging $3.95 per $1,200 of taxes paid for Visa debit transactions.
This makes the $65 card more then pay for itself, in addition to the 5,000 bonus you get for the $65 fee card.
I am heading out now to sign the signature cards at my local WaMu/Chase and they will make a $165 deposit for me to my new Chase account in Arizona.

Excellent find for Californians to harvest 15K miles without travelling to AZ to open a Chase account. If I am not mistaken, on the Visa Debit card you get 1 mile per $1 spent only when you use it as a credit card (without the pin code) and nothing when you use it as a debit card. It would be advisable to re-look at the T&C or talk to the personal banker in AZ before using the card for big payments.
Here's an update on my personal experience:
Enrollment Bonus - 2500 miles (not sure why I received this one / maybe new cardholder?)
Special Bonus - 5000 miles (assuming use of debit card)
6-10 weeks after initial DD will be 6/1/09 (10 weeks) for the additional 10K.
Also, not charged the $25.00 annual fee yet either. Shhhh....

FWIW, the annual fee usually won't be posted until 3-4 months. Trust me, Chase will charge you.
It usually takes a bit longer, sometimes follow-up needed, to see the new account bonus miles. I had to follow up and get my sign up bonuses in the previous years. One time it took almost 6 months to get my bonus miles.
Opened new Chase checking acct. (an additional one to my regular Chase chkg acct) on 3/16/09. Funded it with a couple of hundred dollars cash. Received the UA Mileage Plus Preferred $65 check (debit) card on 3/23.
Used debit card (as a credit card) 5 times on that day in order to avoid monthly checking acct. fee for the month. Had a problem when I tried to use it twice at the same gas station within 5 minutes...card not accepted. Straightened out the problem on the phone with Chase Fraud Prevention.
Today I did the April set of 5 card transactions, but was unable to use it for a 6th "safety" transaction...probably a Fraud issue again. I will wait for them to call.
Also today, exactly a month after my first qualifying transactions, the 5000 UA miles posted.
First genuine direct deposit should occur the first of May. After that occurs, I will tell the payer to change back to my regular Chase checking acct., then wait 6-10 weeks for the 10K miles to post.
Must remember to close the account before 12 months, to avoid the recurring annual $65 check card fee.
