I think most hotel spa/gym sell memberships to the general public. In some hotels, the spa/gym is run by a 3rd party.

I was a member of the One Aldwych Health Club for 4 years and while it was a compact facility it was perfect for my needs. It remains one of my most favorite hotel gyms.
