Thanks, that explains it. Altea appears to award upgrades based on the underlying revenue ticket, ignoring mileage upgrades, and thereby allowing for double upgrades. In your case the system, based on the pcv of your w-ticket and status etc., decided that you should be upgraded from W to I. Since you were in I already, I became O. That logic should be confirmed by some of the reported upgrades of this thread. However, I have not yet experienced/heard of cases where Altea upgraded a straight mileage ticket.

06 SEP LH430 E --> C.
After a multi-year dry spell, that makes two OPUPs in four weeks on this flight. How can I enjoy the great economy plus product if they keep bumping me?

Well, I am not ready to go that far, yet , but I did have an interesting chat with the P1, who was greeting folks at the top of the stairs.
For those who might be interested in increasing OPUP chances: on TATL routes with 2x daily service (such as ORD), according to the P1's observations, the early westbound flights TATL and the late eastbound tend to be more crowded than the other flights, which means higher odds of overbooking and higher odds of bumping people around.
I can neither confirm nor deny that observation with quantifiable facts, but I have no reason to doubt it, either.
