Was caught off guard

I didn't charge my stuff as I thought I'd just use the charger on the train like I did on the westbound trip

well my return train Hiroshima to Tokyo didn't have any outlets

The trains are incredibly punctual--the pressure on the staff must be enormous.

I think it's more a matter of having the right processes in place and that everyone follows them to ensure the success of the operation. JR employees (especially those involved with operating the Shinkansen) take a lot of pride in their work.

Indeed at least one fatal accident has been blamed on excessive pressure on the staff to run on time.

Some of the trainsets have power outlets only at the bulkhead seats at either end of each carriage.

Huh? My understanding was that the shinkansen has never had a fatal accident. Is that not correct?

As far as Shinkansen is concerned, there has been a fatal accident involving the doors closing on a passenger. I think lobster may be referring to trains in general.

Yes, thank you. I assumed the comment I was responding to was about trains in general (in Japan) running on time, so my comment was just about trains in Japan.

Actually I see this pride not just among JR employees, but among all Japanese employees in general, whether it's the staff in the restaurants, hotels, or even just the public services.

Perhaps those who are actually working in Japan may share their view on this.
