I just noticed this on my PC account:

It took way longer than 4 - 6 weeks. I'd forgotten all about the quiz.

Why can't PCR get their act together and fix their craptastic website?
Maybe their have put all of their efforts into having a world class call center? Oh right; that one is staffed by a bunch of kindergarten dropouts as well.

link is accessible, but I'm getting an error trying to submit. Says I need to complete the questionnaire, but it is completed (all questions show answers) but not being accepted.

Yep, I did have that problem at first, but it quickly pointed out the format was wrong and I wasn't able to proceed to the questions. Even after that was corrected, i completed within 2 separate browsers (IE and Firefox) with same result. Seems to get stuck on question 17 -- which is written as though it should even be an optional question.

That was a snap. Thanks for making this survey known. Having lived in Asia a while, I had to get used to putting day/month/year for filling out forms. In US it seemed it was always month/day/year. This might have been some of problems people were experiencing earlier.
