IC San Jose Costa Rica give free access to club and an upgrade on paid stays (no idea about rewards stays)

IC Düsseldorf lets you choose:
Booking Superior Room -> Upgrade to King Club Intercontinental WITH Club Access OR Upgrade to Executive Room.
If you book Club then you will get an Upgrade to Executive Suite.

According to this Please post your Royal Ambassador experiences (Archive Thread 2012)
it seems that if you include breakfast in your reservation the Upgrade will be to Executive Suite anyway + Club Access (from Superior Room).

IC Panama Playa Bonita - had free lounge on 4 Sweet Dilemma free nights a few years back
IC Madrid - had free lounge on Sweet Dilemma free night. Connected to paid stay though
IC Malta - free lounge Nov 2011 on Pointbreaks (PBs).
IC Istanbul Ceylan - free lounge on award Jul 2012
IC Aqaba - free lounge on PBs spring 2011
IC Manila - free lounge on PBs sprin 2012

Club access and suite upgrade given on paid stay at IC Sao Paulo. Hotel is very recently renovated and has very nice club.

Just stayed at IC Jakarta (paid corp rate) and in addition to suite upgrade was told that I could use Club Lounge on 14th floor.

Last year on PBs- no lounge- although I managed to negotiate an exception for the week due to some unclear communication by mail And lounge is decent enough.

IC Asiana Saigon (HCMC) on a paid stay (corp rate) and full access to Club Lounge was granted. RA letter I got actually stated option for breakfast was Club lounge or the Market 39 restaurant on the ground floor.

Room upgrade was also granted. First showed Exec Suite online and when I arrived I got the Presidential suite (yup and I was all alone for 1 night only) which is room 2010. Nice one.

Wow, this is very counter to lots of other RA's experience including me. Did you have to beg/plead to get club access?
