Can anyone please explain what I did wrong? I tried to use several of the referral codes. The first one worked like a charm. Then, I have cleared all cookies and moved to the next booking. But even after I signed up for a new Expedia.uk account, I did not see any box for the promotion code. It directed me all the way to entering my credit card without giving me the promotion option.
Did anyone have the same problems? I have tried using IE, Safari and Firefox and have wasted so many codes. What did I do wrong? Can the codes that were never been entered be used again although I have registered my e-mail addresses as sent by the referrer?

Do note that there are some hotel chains left out of the promotion, notably Hilton and Marriott but also a few others (as per T&Cs). It has to be an Expedia special rate hotel. That would be my first thought. The thing you're looking for is the "I have a voucher" radio buttons in the first screen after you hit the Book It! button (You probably knew that if you've done a successful one, though).
If the code hasn't been "spent" via a bona fide booking that went through it should be reusable. Trying to use multiples from the same account or from different accounts but with the same credit card will get you that "that code has already been used" error even though it hasn't. In some cases it will then add the voucher as a "saved voucher" on the account, but the code hasn't really been burned off. This gets to the heart of the "bug" discussed a lot so far here. Basically the computer code to prevent multiple GBP20 redemptions is also being applied to prevent multiple GBP40 redemptions, even though the T&Cs on the voucher types are different.
I have also found that sometimes, during the booking process, the box to enter a code does not appear. When I get to the last page of the booking process and I've not seen a box, I click on 'change hotel' (an option you get before the payment page), select exactly the same property again, and second time round the 'Got a voucher?' box is normally there.
Thanks a lot to RustyC and Raffles. I have tried Raffles' suggestions couple times more but the voucher box still did not appear. Then, I tried checking the hotel that I previously already booked successfully using the voucher and the box appeared without any issues. So it has to be the hotel, per RustyC's suggestions. However, I was looking to stay at Microtell and according to the terms, this hotel should be ok but it was not. So I just went ahead and booked another hotel. The voucher box appeared right after I clicked the "book it" button.
My other question is that what happens to the code that I have not used? I have registered the referred email address but did not use the voucher. So if I tried to book and have to log in as an Expedia, will my friend get the $40? Does that email address still count as a new customer?

They seem slower for sure in the last few days, and I've also noticed a lag lately between acknowledgement in the friends' list and arrival of the voucher e-mail.

Another tidbit: Figure the booking fee at about 8-10% of the unadjusted cost as a rule of thumb. Maybe GBP2 for a GBP20 otherwise free booking, or 10% of whatever total it was before discount. The voucher doesn't count against this, so you'll be charged *something*. As with Priceline, Expedia appears to bundle its own slug in with the taxes and not provide a specific breakdown, so it's hard to say how much is tax and how much is theirs.
Unlike Priceline, they DO make a good-faith effort to find out and list resort fees at hotels that charge them, so if you're booking in a location where those are rampant (Orlando, Las Vegas, etc.) be sure to read the hotel-decription info. That's always a good idea, anyway.
Make sure you are logged in to Expedia when you apply a voucher... and apply a voucher ONLY if you are going to book.
In my case, after I applied the voucher and proceeded, XP wanted me to log in and then took me back to the previous screen. The price was still discounted but received a "that voucher has been used" message and it would not proceed with the discounted price.
Don't go "shopping" with the voucher, only use it when you are ready to pull the trigger.
I sent my brother a 20 pound voucher on May 29, and he booked a room two days later. I'm still waiting for my 40 pound voucher to show up? This thread suggests that people are getting them faster, if I'm reading it correctly. Should I have gotten my voucher by now?

Yes, I've always received the voucher w/in a day or two. Did he make sure to use the same e-mail address to register on Expedia.co.uk? That's how the vouchers are triggered.
