same here: I was also wondering if anyone figured out how to book flight and hotel in different cities. Some other forums mention that is possible. however, i did not find a way, yet. Is anyone smarter than me?
Did anyone try 2 destinations option for Expedia package?
Doesn't work for me. I tried LGB-SJC-SFO-LGB 9/19-23 selecting the Holiday Inn Silicon Valley for 9/19-22 (which I know works with the promo) and then added one night (9/22) at the Holiday Inn Golden Gateway in S.F. (which also participates in the promo) returning through SFO on 9/23 (all on JetBlue). Promo shows up but says the purchase does not meet the minimum stay restrictions.

Here is an example of how to get the lowest price:
LAX-LAS 9/7-9/10 for 1 person. Select Candlewood Suites which has the $200 off IHG promo listed. Change airline to US Airways flights only (any of the lowest priced flights). Total will be $239.00. Now, taxes are $44.13 as part of the $239.00 total BUT the t&c's of the promo state that your total after the promo has to be higher than the taxes. It won't work UNLESS you select an add on. I added one of the stupid Carbon Footprint add-ons for $6.99. That brought the total to $245.99. Now, the $200 will come off with the code. Total for 3 nights, hotel/air/taxes and carbon footprint add-on $45.99. Now that's a deal.
In lieu of the Carbon Footprint add-on, you can add the "Shop & Shuttle: Fashion Outlets of Las Vegas" option.

Has anyone tested the promo on non-US flights/hotels? All examples so far mention only US carriers and locations.
Yes. So far, all carriers that I've checked, even all US carriers, work towards international destinations. I've only been checking Mexico and the Caribbean originating from the USA, so far, btw.
There are also a few posts on FW and SD that Delta and AA have worked on at least a couple of domestic packages.
Call in to Expedia as a number of posters on FW and SD have done to get this promo to work if you can't online for a participating hotel with your airline of choice. There is no restriction beyond a non-refundable airfare.
I'd love to meet the person who loaded this promo in the system at Expedia and give them a piece of my mind.

I think there's also the restriction of "US customers only". While it does not amount to "US flights/hotels only" in my understanding, I guess not being able to prove my US residence might be the bigger problem.

If you are concerned with just paying for it, based on the Expedia UK promo, it seemed like eveyones cards - from the USA - went though. You can always try.
Another observation on the Expedia/IHG promo, if you have an affiliate (flights flown by Colgan Air or ExpressJet, for example) of the mainline carrier in the mix of your flights, the promo ONLINE will be accepted. I've already tried WAS-EWR on Continental and it does work. I got one down (Newark Holiday Inn) to $44.13 9/24-9/27 by adding a $16.99 carbon green add-on and $1 subscription. Taxes were $43.90 but the entire package was $227 so I had to get the price above the taxes to get the promo to work.
Many people are now calling in and Expedia is adding the $200 coupon to their accounts to book any non-refundable flights on any carrier they wish.
A few hotels have come up such as in Mesa, Az. (Carefree Inn Windemere) and Miami Beach, Fl. (Pelican Hotel) that are NOT IHG hotels but the coupon is working iwth.
