I believe the answer is different whether in advance or at the gate. I believe everyone is on the gate list under their own priority now... and that the GM would trump the FO at the gate... but in advance the FO would trump the GM.

You are incorrect. The lowest status always prevails. FO before GM w/no status companion.
IME, the FO trumps all medallions+non-status companion (if they don't split their PNRs). If they split they split their PNRs, then the GM trumps the FO, and the non-status medallion goes to the bottom of the list. Anyone experience differently?

Let's assume that all are trying for free upgrades from B M H Q K L U T fares based on status. Before the gate, if on the same PNR, the GM and companion clear together after the FO at the companion window. If not on the same PNR, GM clears before FO at GM window, then FO at FO window, then companion at companion window. At the gate, whether or not on the same PNR, passengers are listed by their own status, so it's GM > FO > companion.

They are listed that way but IME they don't clear that way. Many times when traveling with my wife (lowly PM) we are listed on the gate upgrade list separately but in nearly all cases the GA pages me to the podium and asks if I want to split the record so that I can clear the upgrade or if we stay together we won't upgrade. In the few cases when I have not been asked this, the GA just skips me on the list and I ride in Y with her.
GM's don't pay a SDC fee but if I do a SDC to any earlier flight that shows the front cabin wide open does a GM have to pay a change fee for the companion listed on the same PNR?

Technically I believe the answer is yes, but it is often waived.

I just booked a ticket flying with a friend, I wanted to make sure I would get my upgrade and that he had a chance if available - the above is how it was explained to me by the agent.

That is the way it worked for me last week with Mrs. NWDLMEMATL. We were on the same PNR. I am DM and was #2 on the gate upgrade list, she is GM and was like #25.
I believe that is different than it was say 6 months ago.
This only applies when flying with one companion correct? If there is a medallion and 2 or more other people on the same PNR then no one will get upgraded, correct?

When you see your name being skipped on the gate upgrade list, do you try to ask the GA for an explanation? This sounds to me like a case of the GA making up their own rules and somehow dropping you from the upgrade list without asking. Otherwise, the GA shouldn't be able to just skip you easily.

Right. The elite privilege is to upgrade at most one companion.
